Wage:

$24.00 per hour with a $3,000 sign on bonus that includes a two year non-compete contract with external case management providing case management for Black Hills Works participants. This includes RHD and Benchmark Human Services as well as any other company that would provide external case management for Black Hills Works participants.

Hours: Primarily Monday through Friday daytime hours with flexibility as needed.

General Description of the Position:

The Shared Living Specialist (SLS) primary function is to assure that quality services are provided to each individual. The SLS is responsible for the coordination and monitoring of the host home provider relationship and Individual Support Plan (ISP). Their duties include facilitating team meetings, observation and assessment, documentation of goals and supports and maintaining contact between internal operations and external stake holders such as Independent Contractors, Conflict Free Case Managers, family members and community resources.

Qualifications:

* Bachelor’s degree in an education or social service field (or equivalent work experience).
* Previous experience in working with people with disabilities.
* Valid driver's license with a safe driving record - insurable under our insurance.
* Pass a qualifying criminal background check which includes a motor vehicle report.
* Negative drug test.

Responsibilities:

1. Develop trusting and meaningful relationships with host home providers and the Individuals you support.
2. Participate in the host home provider certification process. Serve as a resource by assisting the host home provider with signing up for the necessary training and securing needed paperwork.
3. Complete 3 monthly visits with the host home provider and the Individual. One visit must be in person while the other two may be through phone calls, text message, video chats, etc.
4. Complete required reporting forms each month and upload to Therap.
5. Problem solve support concerns as requested from the host home provider and/or individual supported.
6. Monitor progress of persons individual plan by reviewing TLogs, Case Notes and ISP Data on a weekly basis.
7. Serve as a liaison with the Conflict Free Case Manager. Assist with scheduling ISP and special meetings as required.
8. Complete required assessments and upload into Therap for all team members to review.
9. Create ISP programs for the host home provider and staff members to complete required documentation.
10. Assist with coordination of public transportation when needed.
11. Respond to emergency situations as needed.
12. Document services provided in Therap TLogs and ISP Data.
13. Work with the persons nurse to ensure the host home provider has the necessary medical data sheets for appointments as needed.
14. Review Monthly Invoice from the host home provider and route to BHW Business Office.
15. Route monthly MAR sheets to the host home provider prior to the 1st of each month.
16. Review completed monthly MAR sheets and route to the individuals nurse. Upload the MAR into Therap as required.
17. Monitor finances and complete report requirements.
18. Ensure annual contracts and host home provider certifications are updated as required.
19. Comply with Agency, Health Department and Administrative Rules of SD (ARSD).
